These things are found on BMW R90S carbies for a balance tube to be fitted. They are a press fit item installed opposite the mixture screw. Yes you need to drill a small hole into the carb body for this to work. This procedure is outlines on various BMW forums..... When I find the time I will add the info here.
NOTE FROM A CUSTOMER REGARDING INSTALLATION:
"I just installed a Nipple/Screw Balance Tube Kit - BMW R90S, The hole in the carb body (opposite the idle screw) needs to be enlarged to install the balance tube. A 17/64th bit is required for this, (I also applied loctite 648 to the tube to ensure a good seal). For drilling the hole for the vacuum port, I used a Drill America #57 x 6 in. High Speed Steel Aircraft Extension Drill Bit (from home depot). There was no need to remove the insulator, the cutout for the port was already present, it was just a matter of positioning the tip of the bit in the cutout and aligning the shaft against the carb body intersecting with the seam at the top of the outlet."
